Our Process Begins with You
First, we get to know you. Through meetings, at-home evaluations, and document review, we carefully assess each family or individual situation. We look at legal and financial details and desires, living arrangements and care availability options, and assess access to public benefits like Medicaid. Then, we prepare a unique, fully-developed, understandable legal, financial and social service plan.
Life Care Plan Development
From the evaluation, we create an Action Plan tailored to your specific needs. This Action Plan spells out a cumulative plan to address your legal, financial and social service requirements. Our Elder Care Coordinator is dedicated to your Plan. Your Life Care Planning Agreement assures you of a lifetime commitment of service for one single fee.
Living the Plan
Moving forward, we review your Life Care Plan at least annually. If circumstances require, we update the plan as needed. Of course, you will always have access to your team of professionals for questions, concerns, or assistance in a crisis.
Life Care Plan Components
We will review your estate planning, including creation review and update of Wills, Trusts, Powers of Attorny and Advance Directives. We will review you assets and make suggestions for asset protections. We will assist with long term care planning, from finding care in the home all the way to nursing home care. Will will assist in the preparation of application for public benefits, including Medicaid.
Our Elder Care Coordinator is a licensed clinical social worker and geriatric care manager who specializes in assisting older adults and their families. She will get to know you and help you anticipate care problems, and she will be able to assist in solving those problems. She will help coordinate care with medical and health care providers. She will provide support, guidance and advocacy at all times, and especially during a crisis. She will help coordinate a move into assisted living or skilled nursing.
